Social Services/Care Coordination
Social services staff/care coordinators are available to provide emotional support as well as to answer questions regarding financial concerns, resource information, and post-discharge follow-up. Care coordinators also conduct discharge planning, helping to ensure a smooth transition to a lower level of care. Discharge planning is available to all patients and/or their families. If you would like to speak to a care coordinator, please notify your physician or other staff member who will then make the referral for social services.
Home Healthcare and Hospice
Tucumcari Home Health and Hospice Agency provides skilled care to patients who need specialized medical care and are considered homebound, but do not need the 24-hour monitoring of a hospital or nursing home. We also provide physical, emotional, and spiritual support to terminally ill patients and their families. Our multidisciplinary team helps patients maintain their dignity, comfort, and quality of life in the face of terminal illness.

Rehabilitation Services
We offer a wide range of therapy and support options for patients recovering from strokes, neurological and orthopedic conditions, as well as other illnesses, injuries, chronic conditions, and disabilities. Our therapy services are offered on both an inpatient and outpatient basis. Services include physical, occupational and pulmonary rehabilitation.

Swing Bed Program
The swing bed program provides structured, multi-disciplinary care to help you heal and strengthen before going home from the hospital. A “swing bed” is used when a hospital is able to use the same bed for either acute hospital care or skilled nursing care. Thus, the bed "swings" between the two levels of care. The program helps patients recover when they are no longer acutely ill but need extra time to heal or strengthen before returning home. To qualify for the swing bed program, patients must have:
- Had a three-day acute care hospital stay within the last 30 days and need daily skilled nursing or rehabilitative services treated
- A reasonable expectation for clinical or functional improvement
- A pay source and skilled nursing facility benefit day available through their insurance

Senior Solutions
Senior Solutions is available to adults age 55 and older, who are Medicare eligible. It is an outpatient behavioral health program that offers individuals the opportunity to improve their quality of life by learning to cope with emotional problems, maximizing their ability to function independently. Our services are group-based, focusing on grief/loss resolution, stress management, and family therapy.

Radiology
Services include diagnostic radiology, ultrasound imaging, vascular ultrasound imaging, CT scans, digital mammography, computerized tomography (CAT Scans), bone density testing, nuclear medicine, echocardiography, and magnetic resonance imaging (MRI).
